The number of Australians receiving free general practitioner (GP) visits is on the rise, with national bulk-billing rates climbing significantly. This trend is largely attributed to a substantial government incentive program aimed at reducing out-of-pocket costs for patients. Latest data from the health department reveals an increase in free consultations across all Australian states and territories.

National Bulk-Billing Rate Reaches New Peak

For the June 2026 quarter, the national bulk-billing rate reached 84.1 percent. This marks a notable increase of nearly 4 percentage points compared to the same period in the previous year. The surge indicates a growing number of patients are accessing primary healthcare services without incurring direct fees.

Looking at specific age demographics, nearly four out of every five patients aged between 16 and 64 had their doctor’s appointments bulk billed during the June quarter. This represents a substantial year-on-year increase of 8.2 percentage points. While bulk billing for consultations with the elderly, which are often more complex and time-consuming, has shown improvement, it has not risen at the same pace as other age groups.

These positive figures build upon a significant seven percentage point rise in bulk-billing adoption by GPs since November 2025. This period coincided with the implementation of the Albanese government’s incentive payments designed to encourage doctors to forgo out-of-pocket fees for their patients.

Regional Variations in Bulk-Billing Adoption

The Northern Territory has demonstrated the most rapid uptake of bulk billing, with rates increasing by 13 percentage points in the June quarter to reach 90.8 percent. In New South Wales and Victoria, nearly nine out of every ten patient visits were bulk billed during the same quarter, standing at 87.5 percent and 85.7 percent respectively.

The Australian Capital Territory (ACT) currently shows the lowest bulk-billing rate among states and territories, with 59.3 percent of visits being bulk billed in the last quarter. While this is an increase of 2.4 percentage points, the ACT faces unique challenges. As a more affluent area compared to other regions, it struggles with doctor recruitment to address local shortages and does not qualify for specific regional incentives aimed at attracting general practitioners.

Impact of Government Incentives on General Practices

More than 1400 general practice clinics have transitioned from mixed billing (where some services are bulk billed and others incur a fee) to fully bulk-billing models since the introduction of the government’s incentive program. This shift is a key factor in the government’s commitment to making nine out of ten GP visits free by 2030, a pledge linked to an $8.5 billion investment.

Previously, many GP clinics limited bulk billing to specific groups, such as children and concession card holders. However, the new incentives provide additional payments to doctors who choose to waive out-of-pocket fees for all patients. For a standard consultation in a metropolitan area, this incentive payment is approximately $63.50. Practices that commit to bulk billing all their patients are eligible for an additional quarterly payment equivalent to 12.5 percent of their total Medicare billings.

Concerns and Perspectives on the Incentive Program

Despite the overall increase in bulk-billing rates, some concerns have been raised regarding the program’s implementation and its financial impact on certain practices. Opposition health spokeswoman Anne Ruston suggested during a Senate estimates hearing that some GPs might have felt pressured to adopt bulk billing. She noted that the department’s own figures indicated that approximately 30 percent of practices might be financially disadvantaged by shifting to a full bulk-billing model.

Health department officials presented modelling at the same hearing, suggesting that out of 6600 practices studied, 4800 would benefit from transitioning to bulk billing with the new incentives. This implies that around 27 percent of practices might not see a financial advantage from the change.

Dr. Michael Wright, president of the Royal Australian College of General Practitioners (RACGP), acknowledged the positive impact of the incentives but emphasized that they do not fully cover the operational costs for many practices. He stated that the government’s own modelling suggests the incentives were not designed to make all practices financially viable under a full bulk-billing model.

“General practices are small businesses, you know,” Dr. Wright commented. “No one wins if practices close their doors because they can’t afford to keep their doors open. If politicians really want to help people all around Australia have affordable access to general practice care, the best way to do that is by increasing Medicare rebates, particularly for the longer and more complex consultations, because that’s the care that GPs are increasingly providing for our patients. But the way Medicare is structured, there’s sort of a disincentive for providing that longer care.”

The RACGP advocates for increased Medicare rebates, especially for longer and more complex consultations, arguing that the current structure can discourage GPs from providing extended care. They believe this would be a more effective way to ensure affordable and accessible general practice services nationwide.

Government Response and Future Outlook

In response to these discussions and the evolving healthcare landscape, the government is undertaking a review of the Medicare Benefits Scheme. This review aims to reassess how different types of consultations are costed, potentially addressing some of the structural disincentives mentioned by the RACGP.

Health Minister Mark Butler highlighted the positive outcomes of the increased bulk-billing rates, stating they are beneficial for both patients’ financial well-being and their overall health. “Across the country more and more GP practices are becoming fully bulk-billing clinics meaning all patients need is their Medicare card, not their credit card,” he remarked, underscoring the goal of making healthcare more accessible and affordable.

The ongoing efforts and the current upward trend in bulk-billing suggest a continued focus on improving access to primary care for all Australians, aiming to ease the financial burden of healthcare and promote better health outcomes.
